Hypertensive Emergency
Abstract
Scenario overview A 55-year-old woman with history of end-stage renal disease (ESRD) on hemodialysis, with hypertension, diabetes and coronary artery disease found by neighbors after hearing the patient's dog barking in her apartment for over 2 hours. She is minimally responsive and lying on couch. Neighbor says he spoke to her the previous evening when she told him that she wasn't feeling well and has missed 4 days of dialysis. She arrives by ambulance. Pill bottles were found at beside and brought by EMS to show to the medical team: hydrochlorothiazide, amlodipine, Glucophage (metformin), aspirin. The medical work-up in the ED reveals renal failure, hyperkalemia and CHF. Patient goes into ventricular tachycardic arrest. Team should go through the AMS algorithm and must treat the elevated BP.
