Traumatic Brain Injury
Abstract
Scenario overview A 75-year-old man, PMH hypertension and dementia, was walking his dog with his niece, tripped on a hole in the street while crossing and hit the side of his head on the curb. He had loss of consciousness at the time but recovered and went home, then started to become confused. EMS called 1 hour after the event; man brought in by ambulance on backboard and collar. He is confused and agitated, trying to take off his collar and get off the stretcher, calling for his dog Spotty. Allows IV access but must be sedated for rest of exam and management. After sedation, becomes increasingly agitated and ultimately unresponsive with posturing. He needs advanced airway but is a difficult airway. Head CT shows large epidural hematoma with midline shift. Teaching objectives/discussion points Clinical and medical management.
